Abstract
Background: Sentinel lymph node (SLN) sampling during gynaecological oncology surgery can enable precision surgery, decrease morbidity and improve oncological outcomes. Inspite of many studies, its use in early-stage cervical cancer is relatively recent, with marked heterogeneity in utility across the UK.
Objectives: To explore UK gynaecological oncology experts' current practices, challenges, and attitudes to SLN sampling for the management of early-stage cervical cancer. Secondary objective was to reflect on indications and modalities of SLN sampling especially in the context of fertility-preservation.
Methods: An online survey questionnaire was developed in partnership with a group of UK-based gynaecological oncologists. Following approval by the British Gynaecological Cancer Society (BGCS) survey committee, it was distributed to BGCS members for a six-week period. Thirty-two responses were received in this timeframe.
Results: The survey revealed that 80.8% of respondents use SLN sampling for gynaecological oncology patients within their hospital. Utilization was higher for endometrial and vulval cancers (76.2%), with possible underuse in cervical cancer (66.7%). Barriers included limited familiarity with the procedure and concerns regarding reliability. Only a small proportion of respondents use (open or laparoscopic) SLN sampling alone. Notably, 100% of participants expressed a willingness to transition to this technique. Further efforts need to be directed towards education and training for SLN mapping in early-stage cervical cancer. Furthermore, we propose the creation of a prospective UK surgical database/registry specifically for early-stage cervical cancer to help monitor trends in treatment and identify gaps. Ultimately, this will pave the way for standardization of SLN sampling protocols.
